Your First Visit at Table Mesa Dental
From the moment you arrive, our goal is to make your experience comfortable, welcoming and stress-free—starting with your very first appointment.
Please complete the new patient intake form below and plan to arrive 15 minutes early with a valid photo ID and your insurance card (if applicable). Your visit will take about 60–90 minutes, giving us time to provide a comprehensive oral exam and take digital X-rays to get a complete picture of your dental health.
Dr. Ryan Koenig and our caring team will check for cavities, gum disease, bone loss and signs of oral cancer to ensure you receive the highest level of preventive and diagnostic care.
